CMS invites AMWA members to become partners in Connected Care, a public education campaign to raise awareness of the benefits of chronic care management (CCM) services for Medicare beneficiaries living with multiple chronic conditions. Through this initiative, the Centers for Medicare & Medicaid Services (CMS) Office of Minority Health and the Federal Office of Rural Health Policy at the Health Resources and Service Administration are engaging organizations and health care professionals with support to implement CCM and educate patients about its value.
The CMS Office of Minority Health wants to work with your organization to educate health care professionals about what CCM is, how to implement it in their practices, and how CMS has improved payment for services under Medicare. As a partner, the CMS Office of Minority Health will keep you engaged by sharing updates on CCM, and on its rural and minority health outreach efforts.
You can partner in any of the following ways:
- Sharing the Connected Care Health Care Professional Toolkit, a collection of educational resources to help health care providers implement CCM programs and materials to share with patients. You can view and download these resources from our website at go.cms.gov/CCM.
- Developing a newsletter insert, social media post, or developing a web page on your organization’s site about CCM. You can find templates in the toolkits listed below.
- Distributing information about CCM at an existing event or hosting a Connected Care webinar to provide information about CCM to your members. You can order posters and postcards at no charge to share with health care professionals and patients by visiting productordering.cms.hhs.gov. In addition to these activities, our Connected Care Partner Toolkit offers resources that you can use to raise awareness about the importance of CCM. You can use the toolkit’s educational and media tools on your website, in your communications to members, and at conferences and events – any place where you connect with health care professionals or patients.
